Throughout this course you will be developing a formal, evidence-based practice proposal.

The proposal is the plan for an evidence-based practice project designed to address a problem, issue, or concern in the professional work setting. Although several types of evidence can be used to support a proposed solution, a sufficient and compelling base of support from valid research studies is required as the major component of that evidence. Proposals are submitted in a format suitable for obtaining formal approval in the work setting. Proposals will vary in length depending upon the problem or issue addressed, but they should be between 3,500 and 5,000 words. The cover sheet, abstract, references page, and appendices are not included in the word count.

Section headings and letters for each section component are required. Responses are addressed in narrative form in relation to that number. Evaluation of the proposal in all sections is based upon the extent to which the depth of content reflects graduate-level critical-thinking skills.

This project contains six formal sections: PLEASE SEE THE FEEDBACK FROM PROFESSOR TO MAKE EDITS ON SECTIONS THAT HAS ALREADY BEEN COMPLETED TO COMPILE INTO ON EVIDENCE-BASED PROPOSAL.

Section A: Problem Description- Good background overview of the core problem. Effective consideration of the situation and those driving the change. The PICOT question needed to be clearly stated. Objectives and purpose were present but should be clearly delineated. Good inclusion of content relating to the supporting rationale. Reference list compiled was effective and reflects a good start. You will need to add to this substantially. Be sure APA format is correct.

Section B: Literature Support – Realize that in the final submission, you will need to integrate the various references into a lit review.You provided a good overview of the literature. However, there was no consideration of search methodology, which was a key part of the assignment. Content was well organized and written. Effective consideration of validity. APA format was correct.

Section C: Solution Description – Good consideration of the proposed solution. Overview of the organizational culture just be more tailored to the facility where the study will be conducted. Clear and effective consideration of the expected outcomes of the project. Good consideration of the methods which will be used to achieve the outcomes. Effective consideration of the impact of the outcomes on the overall organization. Effectively written.

Section D: Change Model – Effective and clear consideration of a relevant change model. Good consideration of its implementation throughout the stages of the chosen model. Content was effectively organized and written.

Section E: Implementation Plan – 1. Good description of the setting and your access to potential subjects. 2. Effective description of the amount of time needed to complete this project. Timeline was effective but could be better conveyed in graphic form. 3. Good description of the resources needed in the implementation of the solution. 4. Good description of the methods and instruments to be used for monitoring the implementation of the proposed solution. 5. Effective explanation of the process for delivering the (intervention) solution, along with training requirements. 6. Good outline of the data collection plan. 7. Description of the strategies to deal with the management of any barriers, facilitators, and challenges should be more detailed. 8. Feasibility assessment of the implementation plan not assessed. 9. Description of the plans to maintain, extend, revise, and discontinue a proposed solution after implementation not addressed. 10. Effectively written. 11. APA format correct. 12. Budget not included.

Section F: Evaluation of Process – PENDING FEEDBACK FROM PROFESSOR.

The final paper will consist of the completed project (with revisions to all sections), title page, abstract, reference list, and appendices. Appendices will include a conceptual model for the project, handouts, data and evaluation collection tools, a budget, a timeline, resource lists, and approval forms.

Prepare this assignment according to the APA guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is required for the final paper.
