Patient: Male,  inilials TM 25 yoa, Caucasian. (All findings should be normal) 

☆The student should use the Body System Exam rubric as a guide and only document their physical exam findings.

ʉۢ The student should use medical terminology when documenting the examination findings in the objective section and be specific in documenting each rubric item assessed on video.

 This is not a long narrative transcript of the video. • Please use headers and bullet points for organization. 

• The student should follow APA format with a title page. 

No references are used in a write-up as this is the student’s own physical examination.

Cardio and Peripheral Vascular Examination Video/Documentation check list

“All findings are normal” 

BLOOD PRESSURE:

·         Verbally state to faculty that you would do BP with patient supine and then upright in both arms and compare.

NECK:

·         Neck veins: Jugular venous pressure at 30-45 degrees (demonstrate and state the actual measurement)

·         Carotids: palpate and verbalize findings

·         Carotids: auscultate and verbalize findings

CHEST/TRUNK:

·         Visual Inspection: chest wall abnormalities – sternum

·         Visual Inspection: apical impulse – verbalize if apical impulse pulsations are visible.

·         Visual Inspection: precordial pulsations – verbalize if there are any pulsations visible.

Palpation:

·         Palpation: point of maximum intensity (PMI) – describe

·         Palpation: thrills, leaves, lifts in all auscultatory areas

·         Palpation: palpate abdominal aorta and describe

Auscultation:

·         Auscultate with diaphragm in all four areas A-P-T-M

·         Auscultation: auscultate with bell in all four areas A-P-T-M

·         Auscultation: auscultate with bell/diaphragm at Erb’s point

·         Auscultation: auscultate for physiological splitting of S2 (describe the reason for splitting and best heard on inspiration)

·         Auscultation: auscultate abdominal aorta and describe

·         Auscultation: patient instruction

PERIPHERAL EXAM:

·         Observe all four extremities for color, warmth, presence of abnormalities including edema, varicose veins, shunts.

·         Radial Pulses – describe/grade.

·         Brachial Pulses – describe/grade.

·         Femoral Pulse (auscultate with bell and diaphragm and palpate) – describe/grade.

·         Popliteal Pulse – describe/grade.

·         Dorsalis Pedis Pulse – describe/grade.

·         Posterior Tibial Pulse – describe/grade.

·         Cap refill in fingers and toes.

·         Check fingers for clubbing, cyanosis, splinter hemorrhages, tar staining.
