Enhancing the Sustainability of EBP Initiatives

 

Evidence based practice initiatives accounts for numerous saved lives, improved outcomes, and financial enhancement. One EBP initiative in particular is ventilator associated pneumonia bundle. VAP is a continuous problem for intensive care units, long term care facilities and other areas that care for patients on ventilators. Mechanical intubation negates effective cough reflexes and impedes mucociliary clearance of secretions, causing leakage and microaspiration of virulent bacteria into the lungs. VAP is responsible for 90% of nosocomial infections and occurs within 48 hours of intubation (O’Keefe-McCarthy, Santiago,Lau, G. 2008).

At the hospital I am employed has a care measure bundle in place to prevent VAP. It is a team effort between nursing staff and the respiratory care service. The head of the bed is elevated at 30-45 degrees. In addition, oral care is important, DVT prevention is in place, and finally attempting to wean patient’s off the ventilator occurs at the earliest opportunity.  I would argue Kotter and Cohen’s Model of Change is most appropriate for the VAP bundle utilized at my hospital. There was a sense of “now” regarding the decreasing in these infections. Nurses and other staff were informed of the numbers and the need for change. Educators were involved in the re-education.  Staff was challenged to adhere new standards, reminder posters were placed in appropriate areas, and staff was encouraged to seek assistance as needed.  There was an increase in nurse compliance with the new EBP standards. It is necessary to use infection control programs efficiently and ensure control to prevent or at least decrease VAP frequency. It is necessary to offer training and ensure bundle implementation and participation of all healthcare personnel (Yazici, & Bulut, H, 2018).
